PAYMENTS
Weston Vein Clinic accepts multiple forms of payment, including credit cards, debit cards, cash, insurance, or one of our Care Credit financing plans.
Unless otherwise arranged ahead of time – all payment is due at the time of procedure or office visit. We currently accept Visa, Master Card, Discover and American Express. We apologize, but we do not accept personal checks.
In some cases, your health insurance carrier may provide full or partial coverage for your procedure. We recommend you contact your insurer prior to visiting or scheduling a procedure to check your individual coverage, as it often varies from patient to patient. Our office is happy to submit claims directly to your insurer on your behalf, but you are ultimately responsible for payment in full should your insurer not cover the full fees associated with our services.
FINANCING
We offer NO INTEREST Payment Plans through Care Credit Only
Care credit can be used for any treatment plan costing $300 or more! Our goal is to remain extremely flexible and we will work with you in any way possible to get the results you want and need. If you are worried that payment will keep you from improving your lifestyle, please ask us about Care Credit during your initial visit.
NEW PATIENTS
To ensure a shorter wait time, we ask that new patients print the following forms and fill them out before arrival. Please note that all new surgery patients are required to fill out both the ‘Medical History’ as well as the ‘New Patient Surgery’ forms.
